The Academic Progress Notification System is a tool that allows faculty to communicate academic performance challenges directly to a student and encourages the student to follow up with their professor, advisor, the Office of Academic & Learning Resources (ALR), and additional campus resources.
The system will remain open throughout the semester to allow for early identification of challenges and continued reporting as new challenges arise. This will give faculty and student services staff the opportunity to address students' needs and challenges in a timelier manner. Faculty are highly encouraged to submit an APN form as early in the semester as possible or as soon as challenges/changes are noticeable.
There are two sections of this form: Progress Notification to Student (Part 1) and Campus Referrals (Part 2)
Depending on the nature of the concern, you may opt to complete one or both parts of the form. However, completing either section will notify the Academic and Learning Resources staff.
To submit an Academic Progress Notification, please follow the steps below:
Academic Progress Notification Form
Step 1
This link, stuinfo.rhodes.edu/portal/faculty, provides access to the Academic Progress Notification System (APN) portal. Faculty will use their OneLogin credentials to enter the portal.
Step 2
The Course APN, Advisees APN, and Advising and Planning Enrollment Form tabs will be displayed from the portal dashboard. Click on the appropriate tab to search for a student.
To begin the APN submittal process, click the Submit APN link next to the student's name through the Course APN or Advisee APN tabs. The Academic Progress Notification (APN) form will then be displayed.
Step 3
Faculty will proceed to complete the APN form. Faculty can opt to complete Part 1, Part 2, or both sections of the form, depending on the nature of the concern.
- PART 1: Progress Notification to Student
Completing this section will send the student an email from the faculty member. Please use this section to provide students with constructive feedback, encouragement, and instructions on action steps. If this option is selected, a copy of the APN will be sent to the student’s advisor. - PART 2: Campus Referrals
Faculty can share additional academic information directly with the Office of Academic & Learning Resources that will not be emailed to the student. If faculty want to provide feedback or next steps to the student, this information must be included in PART 1 of the form.
Additionally, faculty can use the APN Form to submit a general referral to campus partners, such as Peer Academic Coaches, Career Services, Athletics, The Buckman Center, and others for additional student outreach.
Step 4
Click the SUBMIT button to send the form.

Non-Academic Concerns
To report non-academic concerns, such as family concerns, housing insecurity, mental or physical health concerns, financial challenges, food insecurity, etc., please use the following forms instead:
o Share a Concern Report: Share information regarding family concerns, housing insecurity, mental or physical health concerns, financial challenges, food insecurity, etc., with the CARE team
o Sex/Gender Discrimination and Sexual Misconduct Reporting Form: Share information about concerns related to sexual misconduct with the Office of Title IX.
o Bias Education Response System (BERS) Reporting Form: Share information about concerns related to harassment or discrimination. The report will be shared with the designated campus office as appropriate.
