Full-time faculty and staff may purchase Life and AD&D coverage for their spouse or same sex partner, as long as they purchase at least an equal amount of coverage for themselves. You may purchase Life insurance for your spouse or partner with a minimum benefit amount of $5,000 and a maximum of $500,000, in increments of $5,000. If you would like Spouse/Partner Life insurance in excess of $25,000 your spouse must complete a health questionnaire and UNUM must approve coverage. Accident insurance is available in increments of $1,000 up to $180.000, with no health questionnaire required.
The premium for Spouse/Partner Life insurance is based on your spouse’s age. The chart below shows the current monthly rates for $5,000 of coverage:

The monthly premium for Spouse/Partner AD&D coverage is $ .033 per $1,000 of coverage.
